Create forms

This article will walk you through how to create custom forms for collecting data from your members

  1. Sign in, navigate to Mezzanine by adding "/admin" to the end of your community URL (Example: yoursite.com/admin)
  2. Click "Pages" from the side menu
  3. Click "Form" from the Add menu
  4. Complete the fields as needed
  5. If you'd only like the form to be visible for a predetermined length of time. fill out the the "Published From" and "Expires On" fields
  6. To have the form display in a menu, it's important to select the menu type from the menu section
To view the data collected by the form: click "Pages" from the side menu, select the desired form, click "View Entries" button or "Select Export CSV"
